Pick-up and Drop off application—Please fill out the application and return application with copies of all required documents to your nearest community center. Households reporting zero income must provide additional documentation. All applications are processed based on the order received.

Step 2: Print and bring your completed application along with required documents.
If all required documents are not received, your application will be determined incomplete; as we cannot process an application without all of the required documents. You will need to reapply. Note: The Agency reserves the right to request additional documents or clarification for any reason deemed necessary.
