DHDS families and community participants will be paid $50 to complete a one-time questionnaire. 

Project SOAR is looking for any parents/guardians/caregivers from diverse backgrounds who:

• Live in Louisiana or have lived in Louisiana in the past 4 years
• Have at least one child under the age of 4
• Have gone to some or all of their child’s well-child check-ups with a doctor or nurse in Louisiana 

Please share your experiences receiving care (prenatal, wellness checks for children 4 and under, and referrals).
